Fluid & TPN Calculator
Total parenteral nutrition in preterm infants requires careful balancing of fluid targets, macronutrient delivery, and glucose load. This calculator integrates fluid targets by gestational age and postnatal day with TPN component volumes — amino acids, lipids, and dextrose — to ensure the caloric and GIR targets are met within the prescribed fluid volume.
TPN volume allocation logic
Amino acid and lipid volumes are calculated from the macronutrient targets, then the remaining fluid volume is allocated to the dextrose solution. If AA + lipid volumes exceed the total fluid target, a warning is shown — this is a common constraint in very fluid-restricted extremely preterm infants.
Note: This calculator provides estimates based on population-level targets. Always individualize TPN orders based on electrolytes, metabolic labs, and clinical status.

GIR per fluid = dextrose g/day × 1000 ÷ (weight kg × 1440 min), summed across all dextrose-containing fluids. Energy: carbohydrate 3.4 kcal/g, protein 4 kcal/g, lipid 10 kcal/g (20%) or 11 kcal/g (10%). Fluid target reference: Adamkin DH; AAP. Pediatrics. 2021;147(1):e2020040267.
Frequently asked questions
How is the glucose infusion rate calculated?
GIR (mg/kg/min) = dextrose% × rate (mL/hr) ÷ (weight in kg × 6).
What are typical neonatal fluid targets?
They rise from roughly 60–100 mL/kg/day on day 1 toward 140–160 mL/kg/day, individualised by gestational age, postnatal day, and losses.
