A Calmer Bath for Early Babies: Why Wrapping Matters
What a 2025 study of 56 premature babies found when nurses compared a "swaddle bath" with a regular tub bath
A new study from a hospital in China found that when babies born a few weeks early were bathed while gently wrapped in a soft towel, they stayed warmer, calmer, and more physically steady than babies bathed the usual unwrapped way in a tub. Far fewer of the wrapped babies cried or clenched their fists, their body temperature dropped less, and their blood flow to the skin stayed better — yet both kinds of bath cleaned the babies equally well. The finding is reassuring and practical: a simple change in how a bath is given, using nothing more than a towel, may make a routine part of newborn care noticeably gentler for the smallest patients [1].
Why Bathing a Premature Baby Is Trickier Than It Sounds
A bath seems like the most ordinary thing in the world, but for a baby born early it is a small physical challenge. Babies who arrive at 34 to nearly 37 weeks of pregnancy — often called "late preterm" babies — look much like full-term newborns but are not as ready for the outside world. Their skin is thin, and their bodies are not yet good at holding on to heat. When they are undressed and lowered into water, they can cool down quickly, and cooling down is not harmless: getting too cold makes it harder for a small baby to breathe steadily, keep a stable heart rate, and use energy well. Hospitals take this seriously, because chilling during ordinary care is common and can be prevented [2], [3]. A bath is also a burst of new sensations — cool air, being unwrapped, water, handling — that can leave a fragile baby stressed, crying, and tense.
How Newborn Bathing Has Changed Over Time
For much of the last century, newborns were bathed soon after birth, quickly and briskly, and little thought was given to how the bath itself affected them. Over the past couple of decades that changed. Nurses and researchers began to treat the bath not as a neutral chore but as something that could either help or harm a baby's delicate skin and stability. Guidance shifted toward bathing babies less often, waiting longer before the first bath, and protecting the skin's natural barrier [4]. Along the way, one debate was about the method itself. Studies compared different ways of washing a baby and generally found that lowering a baby into a small tub of water was gentler than wiping them with a sponge while they lay exposed. But that still left a question: what is the best way to give the tub bath? Out of the movement toward gentler, more comforting care came the idea of the "swaddle bath" — keeping the baby wrapped and curled up, the way they were in the womb, and uncovering only one small part of the body at a time to wash it. Early studies of this approach, including work in China, hinted that wrapped babies cried less and stayed steadier [5], [6]. The 2025 study was designed to test that idea carefully in late preterm babies, measuring many things at once [1].
What the Researchers Did
The researchers ran what is called a randomized controlled trial — the fairest way to compare two approaches, in which each baby is assigned by chance to one method or the other so the groups end up similar and the comparison is unbiased. They enrolled 56 late preterm babies in a newborn intensive care unit and, by a computer's random choice, gave 28 of them a swaddle bath and 28 a regular tub bath [1]. Every baby in the study was medically stable, weighed at least 1,500 grams, and was not on a breathing machine. To keep things fair, both kinds of bath used the same warm water (about 40 °C), the same warm room, and experienced nurses.
In the swaddle bath, the baby was first wrapped in a towel in a curled, tucked position, then slowly lowered into the water up to the shoulders, with the towel opened bit by bit to wash each area and re-covered afterward. In the regular tub bath, the unwrapped baby was supported by the nurse and washed in the water. The nurses measured each baby's temperature, heart rate, breathing rate, oxygen level, and blood flow to the skin, and watched for signs of stress such as crying and clenched hands. They took these measurements at three moments: just before the bath, right after, and 30 minutes later. They also counted the bacteria on the babies' skin before and after, to check that both methods cleaned equally well [1].
What They Found
Right after the bath, the differences were clear. Wrapped babies stayed warmer: their average temperature dipped only slightly, while the unwrapped babies cooled more (a difference the researchers found very unlikely to be down to chance) [1]. The wrapped babies also had steadier heart rates and breathing, and better blood flow to the skin immediately after the bath. The most visible difference was in how upset the babies became. In the tub-bath group, 8 of the 28 babies cried, compared with just 1 of the 28 wrapped babies. Clenched fists — another sign of distress — showed up in 11 tub-bath babies but only 4 wrapped babies [1].
Just as important is what did not differ. Both groups kept healthy oxygen levels throughout, and both baths reduced skin bacteria by about the same amount, so wrapping did not make the bath any less clean [1]. And by 30 minutes after the bath, the two groups had largely evened out — the wrapped babies' advantage was mainly in getting them through the bath itself more smoothly, rather than lasting for hours afterward.
What This Means for Families and Their Baby's Care
For parents, the practical message is encouraging. A gentler, calmer bath is good for a small baby, and this study suggests that a swaddle bath delivers exactly that during the moments that matter most — while the baby is in the water and just afterward. The likely reason is simple and comforting to picture: being wrapped keeps a baby warm by covering more of the skin, and the snug, curled position gives the baby a reassuring sense of being held and contained, much like other soothing techniques nurses use to keep premature babies comfortable and steady [7], [8]. Watching a baby's first baths can be hard for parents when the baby seems cold or upset, so a method that leads to less crying can make those early moments feel better for the whole family, and it is something parents can often learn to do themselves before going home [9].
It is worth being honest about the study's limits, too, because good science comes with caveats. This was a fairly small study done at a single hospital, and the nurses giving the baths knew which method they were using, which can subtly influence what people observe. The researchers measured things like temperature and stress during and just after the bath, but did not follow the babies over weeks or months, so this study cannot say whether swaddle bathing changes anything about a baby's longer-term health [10], [11]. What it does show, quite convincingly, is that a wrapped bath is at least as clean and safe as a regular one, and gentler in the moment — and because it costs nothing beyond a towel, there is little reason not to consider it [12].
It also helps to keep the size of the differences in perspective. The changes the researchers measured were real but modest and short-lived: a fraction of a degree of extra warmth, a slightly steadier heart rate and breathing, and mainly fewer tears during the bath itself. None of the babies in either group became unwell, and both groups were back to similar readings half an hour later. So the swaddle bath is best thought of not as a treatment that changes a baby's health, but as a kinder way to get through an everyday task — a small comfort that adds up when a baby is bathed many times during a hospital stay, and one that fits naturally alongside the other gentle, familiar routines that help premature babies feel secure.
What Researchers Are Working On Next
The natural next step, which the study's own authors call for, is a larger trial across several hospitals, following babies for longer and checking whether the gentler bath prevents actual problems — such as babies getting too cold and needing help to warm up — rather than just measuring comfort in the minutes around the bath [1]. Researchers may also study whether teaching parents the swaddle-bath technique helps babies stay calmer at home. For now, families can take reassurance from a clear and gentle finding: for a small baby, keeping the bath wrapped and warm appears to be a kind and sensible way to make an everyday moment of care a little easier.
